Public Health Nurse II - Clinical/Immunizations Coordinator

Summary

Under the supervision of the Clinical Services Supervisor, the Public Health Nurse II performs a variety of duties in public health nursing programs including family planning, immunization, HIV counseling and testing, STD infection exams, and communicable disease follow-up.   The nurse performs tests and health screening of patients, administers medication, and provides counseling and education on health issues, along with being the Immunization Coordinator.

Clinical Services

  • Performs skilled nursing assessment services and client education in a clinical setting, including but not limited to:

-Family Planning services, options counseling, contraceptive counseling, medical/family assessment

              -Immunization services

              -HIV Counseling and Testing

              -Communicable disease follow-up as needed

              -Basic lab tests and blood draws, point of care testing

              -STI Testing/Treatment

·       Assesses client’s medical needs by performing limited examinations, reviewing and discussing            medical histories, evaluating existing treatments, administrating treatment based on                        department protocols or provider orders in EMR, and monitoring results.

·       Promotes health and prevention of disease in clinic and community settings.

·       Refers clients to appropriate health and social agencies or practitioners.

·       Maintains required documentation and statistical data, and compiles required reports as                   needed.

·       Develop and maintain effective, harmonious and positive work relationships with others.

·       Maintain regular and predictable work attendance.

·       Attend health related workshops as arranged and completes required education and                          certifications.

·       Performs other duties as assigned.

·       Participate in county’s emergency response functions, as assigned.

Immunization Coordinator

  • Understands terms and concepts associated with childhood and adult immunization and the diseases they prevent.
  • Describes and applies current Oregon Health Division Immunization Schedule for specific ages.
  • Understands CCHD policies and procedures for administering vaccinations.
  • Interprets Oregon School Law accurately and implements standards for sites required to report in Oregon.
  • Communicates effectively and accurately with various media outlets.
  • Understands terms and concepts related to Incident Command Systems (ICS) and Emergency Operations Center (EOC) functions.
  • Competent in administrative tasks using various computer programs, teleconferencing, webinars, and use of general office equipment for the immunization program.
  • Interviews each parent/client for possible precautions and contraindications specific for each vaccine and possible reactions.

     Vaccine Administration

  • Describes and demonstrates proper vaccine storage and handling.
  • Administers vaccine in a safe manner using universal precautions and sharps safety according to standing orders and procedures.
  • Demonstrates safe child comfort restraint and immunization technique per CCHD policy.
  • Describes procedures accurately for immunization related emergencies.

Documentation

  • Completes all forms, consents, records, and other documentation as required by Crook County and Oregon Health Division.
  • Competently documents in electronic medical record software. Understands terms and concepts involved in determining payer source for vaccine with local, state and federal programs or private insurance.

       Program Development

  •    Creates and implements clinic policies and procedures.
  •    Collaborates and develops community partnerships.
  •    Promotes educational outreach efforts to increase immunization rates in Crook County.
  •    Participates in data collection for local, state and federal programs.
  •    Maintains regional partnerships and coalition participation.
  •    Pursues grant writing opportunities for program promotion.
  •    Coordinates mass immunizing clinic events including logistics, organizing mass transportation,       security, parking, traffic flow, emergency staff and accommodations for public.
  •    Attends continuing education re: changes in vaccine epidemiology.
  •    Participates in the AFIX Immunization Project and metrics.

Medication

  •  Describes and demonstrates proper dispensing of pharmaceuticals.
  •  Administers medications in a safe manner using universal precautions and sharps safety   according  to standing orders and procedures.
  •  Describes procedures accurately for clinic related emergencies.

Laboratory

  •    Demonstrates proper specimen handling, processing, testing, charting and disposal.
  •    Demonstrates proper use of lab equipment, calibrating and documenting controls, using               testing  mediums and processes.

General

  •    Assist in the clinical area performing RN functions as needed during the walk-in clinics as             assigned.  This may include immunizations.

Competencies

1.      Equipment used – Personal computer and related software, telephone, printer, copier,                       facsimile.

2.      Knowledge of principles and practices of nursing and public health nursing including health                 maintenance and disease prevention; the effects of physical, social, and emotional factors on            health; principles and techniques of teaching, counseling, interviewing, and history taking;                 resources available through other health and welfare agencies; integration and linkages                     between community agencies. 

3.      Knowledge of prenatal care and child development.

4.      Ability to exercise independent judgment and plan, organize, and administer safe and                       effective  public health nursing care; work effectively with individuals and families to assist               them in the satisfactory solution of health programs; recognize and respect the impact of                   cultural, ethnic, and linguistic difference on health care practices; act as a liaison with other            agencies to coordinate care of clients; maintain effective working relationships; maintain                  complete and accurate records and reports; evaluate situations accurately and adopt an                    effective course of action.

Supervisory Responsibility

  This position has no supervisory responsibilities

Work Environment

This is largely a clinical role, with some sedentary work; however some filing, lifting and

carrying may be required. This role routinely uses standard office equipment such as

computers, phones, photocopiers, filing cabinets and fax machines. Ability to work in a fast

paced environment and off-site for clinics.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit; stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms.  Ability to lift up to 20 pounds, twist, and bend.    Ability to perform CPR if needed.

Position Type and Expected Hours of Work

This is a full-time position. Days and hours of work are Tuesday through Friday, 8:00 a.m. to 5:00 p.m.

Travel

There is occasional travel for conferences and regional meetings.

Required Education and Experience

  • Degree Required:  Bachelor of Science Degree from an approved school of nursing; or Associate Degree in Nursing with a valid Nursing License
  • Experience in Public Health or combination of experience and training.

Preferred Education and Experience

  • Bachelor’s Degree in Nursing or Associated Degree with a Bachelor’s in a related field.

Additional Eligibility Requirements

  • Must hold a valid Oregon driver’s license and valid American Heart Association CPR card.
  • Prior to or on date of hire:
    • Able to complete a thorough employment and/or criminal history check.
    • Able to successfully pass a pre-employment drug test (not required for all positions).
